There are many different groups or systems that caregivers of an individual with a disability have to keep track of. The IEP team is one such group.

Your IEP team usually consists of the school principal, a special education teacher, a general education teacher, the caregiver, the school therapists, the child (optional), and an advocate(optional).

This sheet will give you an easy way to track who these people are and how to contact them.Remember always to put your requests in writing. If you have a conversation with your child’s teacher, always follow-up with an email. You will be able to log everyone’s email on this sheet, making it easy for you to contact your child’s teachers and therapists.
